# Inschrijven nieuwe patiënten / Subscription new patients

**Wij nemen weer nieuwe patiënten aan ! Houd er wel rekening mee dat we het inschrijfkanaal soms sluiten wegens grote drukte.** 
  
**Belangrijk:** Je kunt je alleen inschrijven als je aan één van de volgende voorwaarden voldoet:  
• Je woont op **hetzelfde adres** als een **actieve patiënt** van ons.  
• Je **postcode** valt binnen onze **postcode-selectie** (zie hieronder):  
  
**Den Haag**  
2595, 2511 t/m 2516, 2521 t/m 2526, 2592, 2593, 2595.  
  
**Rijswijk**  
2281, 2282.  
  
**Voorburg**  
2272, 2273, 2274, 2275.  
  
**Leidschendam**  
2261, 2265, 2267.  
  
**Wil je je inschrijven ?**  
  
Klik op [Mijn Praktijk](https://de-nieuwe-dokter.mijnpraktijk.nl/?ref=nieuwedokter.mymagic.page) om je eenvoudig online in te schrijven. Het kan zijn dat we de lijn **tijdelijk sluiten** wegens het hoog aantal aanmeldingen. Na het inschrijven, gooien wij de lijn weer open voor nieuwe aanmeldingen. **Houdt hierbij de website wekelijks in de gaten.**  
  
Wij verwerken de aanmeldingen wekelijks in volgorde van binnenkomst. Houd er rekening mee dat het **ongeveer 1 tot 2 weken** kan duren voordat je officieel bij ons bent ingeschreven.  
  
Na de inschrijving meld je jouw **uitschrijving bij je vorige huisarts**, zodat wij jouw **medisch dossier** kunnen ontvangen.  
  
• **Let op:** Wij maken geen uitzonderingen op de postcodegrenzen — dit is in verband met veiligheidsredenen. Dit geldt ook voor voormalige patiënten die zich willen her-inschrijven.  
• **Let op:** Dokter Kant ziet op dit moment geen nieuwe patiënten.

**Important:** You can only register if you meet one of the following requirements:  
• You live at the **same address** as an **active patient** in our practice.  
• Your **zipcode** falls within our **zipcode selection** (see the selection above):  
  
**How to register?**  
  
You can click on [Mijn Praktijk](https://de-nieuwe-dokter.mijnpraktijk.nl/?ref=nieuwedokter.mymagic.page) and fill out the form online. The line can be **temporarily closed** due to the high amount of subscriptions. We are registering weekly, once the waiting list is shorter we will open the subscription line again for registration. **Please keep an eye on the website.**  
  
  
We process registrations weekly in order of receipt. Please note that it may take **approximately 1 to 2 weeks** before your registration is complete.  
  
**After registration**: Please inform your **previous GP** that you are registering with us so we can receive your **medical records.**  
  
• **Please note:** We **do not make exceptions** to our zipcode selection for **safety reasons**. This also applies to **former patients** wishing to re-register.  
  
• **Please note:** Doctor Kant is currently **not seeing new patients.**
